Pressure Injuries: Stages 1–4

A simplified educational view of how clinicians classify pressure injuries by the depth and extent of tissue involvement.

Stage 1Intact skin with localized nonblanchable redness.
Stage 2Partial-thickness skin loss with exposed dermis.
Stage 3Full-thickness tissue loss extending into the subcutaneous layer.
Stage 4Full-thickness skin and tissue loss with deeper structures involved.

Key mechanism: prolonged pressure, or pressure combined with shear, can reduce blood flow and injure skin and underlying soft tissue.
